It should also be mentioned that submitting a ban request is dangerous. Not only are you asking us to prevent a player from playing on the server, but you, the poster, can suffer consequences if we find that the request was posted for an invalid reason. Keep this in mind before and during the posting process.
As mentioned above, the request [b]MUST [/b]follow the following template and all of the fields are [b]REQUIRED[/b]:

The player in question's Steam name:
The player in question's Steam profile link:
The player in question's offense:
Date/Time of the event:
Evidence (Photo evidence is preferred. Video evidence is even better.):

If you have video evidence then please post it on YouTube and make the video private so that only someone with a link can view it.
